RESEARCH ON ORAL CANCER SURGERY WITH RECONSTRUCTIVE DEFECTS BY NASOLABIAL FLAP

Abstract
Introduction: Oral cancer accounts for 30% of total head and neck cancers and 5% of all cancers. Surgery for tumor removal must ensure complete excision with a safe margin to minimize recurrence, but it often leaves significant defects. The nasolabial flap is widely used for defect reconstruction, offering advantages such as a high flap survival rate and flexibility. Objective: To evaluate the surgical outcomes of oral cancer with defect reconstruction using the nasolabial flap at Otolaryngology Department – Cho Ray Hospital from January 2020 to october 2023. Subjects and Methods: A non-randomized clinical intervention study was conducted. The study involved 32 cases of oral cancer surgery using the nasolabial flap for defect reconstruction in the first phase. Results: The tumors were located in the floor of the mouth (10 cases), tongue ± the floor of the mouth (21 cases), and hard palate (1 case). The average size of the nasolabial flap was 2.7cm (2.5-2.9) x 5.8cm (5.5-6 cm) with a static venous pedicle. Patients resumed oral intake after 5 days. No tumor recurrence was observed after 6 months, and oral function returned to normal. Complications: paralysis of mandibular nerve grade III-IV (7 cases), which recovered after 6 – 8 months; facial paralysis grade III (5 cases) and grade IV (2 cases). Conclusion: The nasolabial flap is a good option for reconstructing moderate defects following oral cancer surgery
