Abstract

Background: Acute cholecystitis due to gallstones in elderly patients is often challenging to diagnose because of commonly associated comorbidities. Objectives: To describe the clinical and paraclinical characteristics of acute cholecystitis due to gallstones in elderly patients at Can Tho City General Hospital. Materials and methods: A cross-sectional descriptive study was conducted on 61 patients over 60 years old diagnosed with acute cholecystitis due to gallstones at Can Tho City General Hospital from January to October 2023. Results: The male-to-female ratio was 1:2.38; 50.8% were overweight or obese; 44.3% had a history of internal medical conditions. Clinically, 83.6% of patients were admitted to the hospital within 72 hours of symptom onset; the most common pain site was the epigastric region (75.4%); pain was dull or severe in 57.4% and 31.1% of cases, respectively; 77% of patients had body temperatures below 38°C. Paraclinical findings included 70.5% with white blood cell counts >10,000/mm³; gallbladder wall thickness >6 mm on ultrasound in 80.3% of cases; and CT scans showing gallstones obstructing the gallbladder in 100% of patients. Conclusion: Acute cholecystitis due to gallstones in elderly patients primarily occurs in females and overweight individuals, with the most common symptom being dull pain in the epigastric region. Diagnosis is confirmed through ultrasound and CT imaging.
