Mục tiêu: Hội chứng Duane thuộc nhóm các hội chứng co rút bẩm sinh thường gặp nhất với nhiều đặc điểm lâm sàng phong phú. Trong đó, hiện tượng rối loạn vận nhãn theo chiều đứng hay còn gọi là overshoot gặp ở 25 – 39% số trường hợp. Bài báo này giới thiệu 2 ca mắc hội chứng Duane được phẫu thuật lùi cơ trực ngoài chữ Y để điều trị tình trạng overshoot. Đối tượng và phương pháp nghiên cứu: 2 ca lâm sàng bệnh nhân mắc hội chứng Duane đã được phẫu thuật điều trị overshoot. Ca thứ nhất được lùi cơ trực ngoài kết hợp chẻ cơ chữ Y. Ca thứ hai được lùi cả hai cơ trực ngang kết hợp với chẻ cơ trực ngoài chữ Y. Kết quả: Theo dõi sau 1 năm, bệnh nhân đã được cải thiện tình trạng co rút nhãn cầu và không còn overshoot. Kết luận: Điều trị được cá thể hóa với từng trường hợp. Kỹ thuật lùi cơ trực ngoài kết hợp với chẻ cơ chữ Y rất hiệu quả trong việc điều trị tình trạng overshoot ở hội chứng Duane.
