Abstract

Objective: To evaluate the early outcomes of surgical resection of oropharyngeal squamous cell carcinoma via mandibular approach with reconstruction using anterolateral thigh flap. Patient and Methods: A descriptive study involving 14 patients with T3-T4aNxM0 stage oropharyngeal squamous cell carcinoma treated at the Oncology and Head-Neck Surgery Center of the National ENT Hospital and the Maxillofacial Plastic Surgery Department of Viet Duc Hospital from January 2020 to November 2022. Results: The tumor resection rate at the microscopic level reached 100%. All grafts had good viability and provided sufficient coverage for the defected areas. No patients died during or after surgery. Postoperative complication rates were low, with the most common being surgical site infection (57.1%), followed by postoperative bleeding (7.1%), oropharyngeal fistula (7.1%), lymphatic fistula, and mandibular injury (0%). These complications did not affect the timeline for postoperative radiotherapy. Conclusion: Surgical resection of oropharyngeal squamous cell carcinoma via mandibular approach proved effective, with good control of complications and broad tumor resection. The 100% success rate of reconstruction also highlights the superiority of the anterolateral thigh flap in oropharyngeal reconstruction.
