ANALYSIS OF DIRECT MEDICAL COSTS IN OUTPATIENT STROKE TREATMENT AT THONG NHAT GENERAL HOSPITAL - DONG NAI, 2024

Abstract

Background: In the context of limited healthcare budgets, evaluating direct medical costs (DMC) in the treatment of economically burdensome diseases, particularly stroke, is essential. Objective: To analyze the DMC of outpatient stroke treatment at Thong Nhat General Hospital – Dong Nai in 2024.. Methods: A cross-sectional descriptive study based on retrospective data of outpatient visits for stroke patients who met the inclusion criteria at Thong Nhat General Hospital – Dong Nai in 2024. Results: A total of 1,903 patients with 9,051 outpatient treatment episodes were included. The male-to-female ratio was 1.2:1, and the mean age was 63.39 ± 11.61 years. The average DMC per outpatient episode was 470,627 VND (95% CI: 456,949–484,305 VND), with health insurance covering the majority of the cost (89.5%). Analysis of associated factors showed a positive correlation between age, number of comorbidities, and outpatient DMC (r > 0; p < 0.05). Higher costs were found among patients residing in rural areas, with 80% insurance coverage, those insured under social insurance, diagnosed with unspecified stroke, having diabetes, admitted for emergency reasons, and with unchanged treatment outcomes (p < 0.05). Conclusion: Compared to the 2024 GDP per capita, the cost of one outpatient stroke treatment episode accounted for 0.41%. Factors associated with outpatient stroke treatment costs included age, number of comorbidities, place of residence, health insurance coverage level, type of insurance beneficiary, principal diagnosis, comorbid hypertension, and reason for hospital visit.
