EVALUATION OF THE EFFECTIVENESS OF COMBINED INTRAVITREAL ANTI-VEGF INJECTION AND SUBTHRESHOLD MICROPULSE LASER IN THE TREATMENT OF PERSISTENT DIABETIC MACULAR EDEMA

Abstract

Objective: To evaluate the efficacy and safety of combining intravitreal Anti-VEGF injections with subthreshold micropulse laser (SMP) in the treatment of persistent diabetic macular edema (DME). Methods: A prospective descriptive study was conducted on 38 eyes of 38 patients diagnosed with persistent DME at Hanoi Eye Hospital. All patients were treated with intravitreal Anti-VEGF injection combined with SMP directed by OCT findings. Key indicators such as best-corrected visual acuity (BCVA), central retinal thickness (CRT), and central average thickness (CAT) were collected and compared before and after treatment. Results: After 3 months of treatment, BCVA improved from 0.61 ± 0.33 to 0.47 ± 0.34 LogMAR; CRT decreased from a mean of 430.2 ± 129.1 µm to 365.2 ± 136.3 µm; and CAT decreased from a mean of 432.8 ± 104.3 µm to 375.7 ± 111.4 µm. No central scotoma or macular scarring was recorded. Conclusion: The combination of SMP and intravitreal Anti-VEGF injection is a safe and effective treatment strategy for patients with persistent DME who respond poorly to Anti-VEGF monotherapy.
