RECURRENT STROKE IN OLDER ADULTS: SITUATION AND ASSOCIATED FACTORS AT THE EMERGENCY AND STROKE DEPARTMENT, NATIONAL GERIATRIC HOSPITAL

Background: Stroke recurrence is a significant concern in elderly patients due to its impact on morbidity and mortality. Identifying associated factors can help improve secondary prevention strategies. Objectives: To describe the clinical and epidemiological characteristics of patients with acute ischemic stroke and identify factors associated with stroke recurrence. Methods: A cross-sectional study was conducted on 146 patients with ischemic stroke. Data on demographics, medical history, behavioral risk factors, and treatment adherence were collected. Stroke recurrence was defined as ≥2 episodes. Associations between risk factors and recurrence were analyzed using odds ratios (OR) with 95% confidence intervals (CI). Results: The mean age was 79.1 ± 10.7 years; 56.2% were male. Hypertension was present in 68.8%, diabetes and dyslipidemia each in 31.2%, and coronary artery disease in 13.9% of patients. Treatment adherence was high (94.7%). Stroke recurrence occurred in 41.8% of patients. Hypertension (OR = 2.58; 95% CI: 0.86–7.79) and coronary artery disease (OR = 2.37; 95% CI: 0.59–9.52) tended to increase recurrence risk. Other factors such as diabetes, atrial fibrillation, male sex, low education, and rural residence were more common among patients with recurrent stroke. Conclusions: Stroke recurrence is common among elderly patients, particularly those with hypertension and coronary artery disease. High treatment adherence is encouraging, but continued management of modifiable risk factors is necessary to reduce recurrence risk.
