Tóm tắt

Mục tiêu: khảo sát các yếu tố ảnh hưởng đến tỷ lệ phát hiện HLG và tỷ lệ di căn HLG trên BN ung thư dương vật (UTDV) cN0 nguy cơ cao. Đối tượng, phương pháp: mô tả loạt ca 50 trường hợp carcinôm tế bào gai dương vật T2-4 cN0 hay Grad 3 được sinh thiết hạch lính gác (STHLG) dưới hướng dẫn của Tc-99m và so sánh với tình trạng hạch bẹn sau khi nạo. Kết quả:  Tỷ lệ nhận diện HLG là 94% theo vùng bẹn (100% theo bệnh nhân) và không liên quan đến tuổi, BMI, kiểu ăn lan của bướu, kích thước bướu, cT, hạch vùng sờ chạm trên lâm sàng, trình trạng nghi di căn hạch vùng trên lâm sàng hay siêu âm. Tỷ lệ di căn HLG là 10% số BN. Tình trạng di căn HLG liên quan có ý nghĩa thống kê với tuổi ≤ 40, bướu ăn lan kiểu thẳng đứng, kích thước bướu ≥ 5cm, xếp hạng lâm sàng yếu tố bướu cT3, hạch vùng nghi di căn trên lâm sàng hay trên siêu âm
