Abstract

Background: Non-alcoholic fatty liver disease (NAFLD) is a common condition in individuals with type 2 diabetes, especially among the elderly. However, in Vietnam, specific data on this population remain limited. Objective: To determine the prevalence of NAFLD and identify associated factors in elderly patients with type 2 diabetes. Methods: A cross-sectional study was conducted at the Endocrinology Outpatient Clinic, Thong Nhat Hospital, from August to December 2024. Participants were patients aged ≥60 years with type 2 diabetes. Results: Among 404 participants, the prevalence of NAFLD was 54.2%, with 37.3% having mild steatosis, 12.3% moderate, and 4.6% severe. Multivariate logistic regression analysis identified independent factors associated with the severity of NAFLD, including female sex (OR = 0.38; 95% CI: 0.23–0.63; p < 0.001), waist circumference (OR = 1.08; 95% CI: 1.03–1.12; p = 0.002), HbA1C (OR = 1.52; 95% CI: 1.21–1.90; p < 0.001), and triglyceride levels (OR = 1.39; 95% CI: 1.13–1.71; p = 0.002). Conclusion: NAFLD is highly prevalent among elderly patients with type 2 diabetes. Female sex, increased waist circumference, elevated HbA1C, and higher triglyceride levels were independently associated with greater severity of hepatic steatosis.
