Đặt vấn đề: Suy hô hấp là nguyên nhân tử vong hàng đầu ở trẻ sơ sinh và thường gặp hơn khi tỉ lệ sinh mổ tăng. Việt Nam chưa có nhiều báo cáo về suy hô hấp sơ sinh sau sinh mổ.
Mục tiêu: Xác định tỉ lệ và các yếu tố liên quan suy hô hấp sớm ở trẻ sinh mổ ≥ 35 tuần tuổi thai.
Phương pháp nghiên cứu: Nghiên cứu bệnh – chứng tiền cứu trên 257 trẻ sinh mổ ≥ 35 tuần tuổi thai, gồm 57 trẻ suy hô hấp sớm trong vòng 24 giờ sau sinh cần hỗ trợ hô hấp trên 2 giờ và 200 trẻ khỏe mạnh. Phân tích hồi qui logistic được sử dụng để xác định các yếu tố liên quan.
Kết quả: Tỉ lệ suy hô hấp sớm là 3,8% tổng trẻ sinh mổ từ 01/09/2023 đến 30/07/2024. Các yếu tố liên quan suy hô hấp gồm mẹ mổ gây mê (OR = 3,36; 95% CI: 1,38–8,14), tuổi thai thấp (OR = 0,49; 95% CI: 0,36 – 0,69): cụ thể 35-36 tuần (OR=9,58; 95% CI: 3,35–27,38), 37-38 tuần (OR=2,07; 95% CI: 1,02– 4,18).
Kết luận: Tỉ lệ trẻ suy hô hấp sớm sau sinh mổ là 3,8%. Tỉ lệ này càng cao khi mẹ mổ gây mê và tuổi thai càng thấp. Vì vậy cần xử trí mổ lấy thai và xây dựng hướng dẫn theo dõi phù hợp tại các trung tâm sản và sơ sinh.
