ĐÁNH GIÁ KẾT QUẢ CẮT LẠNH POLYP DƯỚI 10MM, SO SÁNH VỚI CẮT POLYP TIÊU CHUẨN

Mục tiêu: Đánh giá bước đầu cắt Snare lạnh polyp đại trực tràng kích thước 0,5cm – 1,0cm. Phương pháp nghiên cứu: Nghiên cứu mô tả tiến cứu trên 106 bệnh nhân Trung tâm Tiêu hóa Gan mật bệnh viện Bạch Mai thời gian từ tháng 05/2022 đến tháng 08/2022. Kết quả: 54/106 người bệnh được thực hiện phương pháp cắt polyp lạnh. Kích thước trung bình polyp của cắt lạnh nhỏ hơn so với cắt nhiệt (5,80±1,30 với 7,67±1,82, p<0,0001). Cắt lạnh chủ yếu thực hiện ở các tổn thương bề mặt, hơi lồi -IIa (79,6%). 100% trường hợp cắt lạnh không cần tiêm nhấc hỗ trợ so nhóm cắt nhiệt tỷ lệ là 78,9% (p<0,0001). Kẹp clips ngăn biến chứng chảy máu, thủng ở nhóm cắt nhiệt cao hơn so với cắt lạnh (38,5% với 3,7%, p<0,0001). Trong nghiên cứu không gặp biến chứng chảy máu muộn, thủng sau cắt polyp. Kết luận: Cắt lạnh polyp đại tràng an toàn ở nhóm polyp nhỏ (5-10mm), ít cần dùng dụng cụ tiêm nhấc, clips hơn nhóm cắt nhiệt.
