LOWER EYELID BLEPHAROPLASTY THROUGH TRANSCONJUNCTIVAL APPROACH: A CLINICAL CASE REPORT

Abstract

Objectives: Lower eyelid blepharoplasty is relatively complex because it involves structures assoctiated with both eyes and the midface. Lower eyelid fat pads are a common problem, related to the aging process of the face but can sometimes be observed in young individuals as well. There are currently limited reports in Vietnam on the application of the transconjunctival approach to remove lower eyelid excess fat, especially in young patients. This method leaves no skin scars, has few postoperative complications, and achieves great outcomes in both aesthetic and functional aspects. Case report: A 28-year-old female patient came to the aesthetic clinic because of puffiness in both lower eyelids when she was a child, with no special medical history recorded. The patient's face shows no signs of aging but there are fat pads in the medial part of lower eyelids on both sides. The transconjunctival approach is chosen to remove the fat pads behind the orbital septum. Great aesthetic outcome can be seen immediately after the surgery. Discussion: Aesthetic surgical procedure of lower eyelids through the conjunctival approach has been reported for a long time, has been technically improved, and widely applied with various indications. This method has been proven to be effective in terms of aesthetic perspective for patients, avoiding unwanted complications of the traditional transcutaneous approach. Conclusion: Lower eyelid blepharoplasty performed by the transconjunctival approach to remove fat pads is a safe and highly effective method, especially for young patients with little or no excess eyelid skin.
