Objectives: Evaluating the effectiveness of the stair step incision technique close to the eyelid margin in lower eyelid surgery. Subjects and methods: Prospective study, uncontrolled clinical intervention on 95 eyes of 54 patients who had lower eyelid surgery at Nghe An Eye Hospital from June 2022 to December 2023. Results: The stair step incision technique close to the eyelid margin in lower eyelid surgery used in surgery to remove fat bags, remove excess lower eyelid skin and move the lower eyelid fat bag to fill the tear trough. With this technique, the skin incision close to the free margin right below the eyelashes helps limit and cover post-operative scars and the stair step incision helps avoid ectropion complications compared to other techniques. Results from the study showed that in 3.7% of operated eyes, scars were clearly visible and gradually disappeared after 3 months of surgery. The average postoperative time of swelling and bruising is 6.15 days; Postoperative complications were ectropion 2.1%, conjunctival edema 7.3% and dry eye 11.6%. These complications improved spontaneously and disappeared after 3 months without surgical intervention. The overall assessment of postoperative results was 84.2 good after 1 month and increased to 92.6% after 3 months. Research results also show that there is a statistically significant relationship (P < 0.05) between age and reason for surgery, the older age group complains about fat and excess skin on the lower eyelid while the younger group complains about the amount fat bag to fill the tear trough. will pay attention to the imbalance of the fat area under the eyelid skin. The average age of choosing surgery is 49 years old, and the age of choosing surgery in men is statistically significantly higher than in women
