EVALUATION OF BREAST RECONSTRUCTION USING DIEP OR TRAM FLAP AFTER MASTECTOMY AT CHO RAY HOSPITAL

Abstract

Breast reconstruction after breast cancer removal is a necessity for many patients with breast cancer, especially young patients. As socio-economic conditions develop, the demand for breast aesthetics after being removed due to cancer is increasing to improve the quality of life and aesthetics of patients. Objectives: evaluate using TRAM and DIEP flap for breast reconstruction after mastectomy due to breast cancer removal at Cho Ray Hospital. Patients and study’s method: retrospective cross-sectional description of 28 cases of post-mastectomy patients reconstructed with TRAM flaps or DIEP flaps between 01/2014 and 12/2023 at Cho Ray Hospital. Results: the average age of the study sample was 45.9±8.9 years, with 32.1% of patients having a BMI of >25. Of the 28 cases of breast reconstruction, 18 were instantly reconstructed and 10 were delayed reconstructed. There were 18 cases using DIEP flaps (64.3%) and 10 cases reconstructed using TRAM flaps (35.7%). There were 2 cases of partial flap necrosis and 1 case of total flap necrosis. After surgery, the aesthetic classification of breasts was good 57.2%, moderate 32.1%, average 7.1% and poor 3.6%. The high and moderate satisfaction rates were 28.6% and 42.9%, respectively. Conclusion: breast reconstruction using TRAM and DIEP flap after breast cancer removal improve breast aesthetic function with high patient satisfaction rate.
