COMPLICATIONS AND SOME RELATED FACTORS OF ACUTE ISCHEMIC STROKE TREATED WITH INTRAVENOUS THROMBOLYSIS AT THE NEUROLOGY CENTER, BACH MAI HOSPITAL

Abstract
Objectives: To describe the complications and some related factors associated with acute cerebral infarction treated with intravenous alteplase at a dose of 0.9 mg/kg. Methods: A cross-sectional descriptive study was conducted by enrolling 40 patients diagnosed with acute ischemic stroke who received intravenous alteplase at the Neurology Center, Bach Mai Hospital, from January 2023 to May 2024. Results: Among the participants, 67.5% were male, with an average age of 64.6 ± 10.0 years. Comorbidities included hypertension (65.0%), diabetes (20.0%), hyperlipidemia (7.5%), and a history of cerebral stroke (10.0%). The rate of hemorrhagic complications was 22.5%. Other complications included symptomatic cerebral edema (5.0%), acute heart failure (5.0%), and pneumonia (7.5%). One patient experienced hypersensitivity to alteplase (2.5%). Factors related to bleeding complications included a history of diabetes and systolic blood pressure ≥140 mmHg (p<0.05). Conclusion: The most concerning complication when treating with intravenous alteplase for acute ischemic stroke was intracranial hemorrhage.
