COMPARISON BETWEEN INTRACORPOREAL AND EXTRACORPOREAL ANASTOMOSIS IN LAPAROSCOPIC COLECTOMY

Abstract

Aims: This study aim was to compare the early results (operative time, estimated blood loss, number of harvested lymph nodes, intraoperative and postoperative complications, postoperative recovery…) as well as the long term results (recurrence rate, distant metastases rate…) between intracorporeal and extracorporeal anastomosis in laparoscopic colectomy for colon cancer. Methods: Retrospective corhort study. From 01/2021 to 8/2022, there were 94 patients, divided into two groups: intracorporeal anastomosis (n = 45) and extracorporeal anastomosis (n = 49). The operative time, estimated blood loss, number of harvested lymph nodes, intraoperative and postoperative complications, postoperative hospital stay, time to first flatus, and long term results were compared between two groups. Results: The intracorporeal anastomosis group has significantly shorter time to first flatus and shorter postoperative hospital stay. Although the anastomosis forming time is significantly longer in the intracorporeal anastomosis group, the total operating time is comparable between two groups. There was no difference in estimated blood loss, number of harvested lymph nodes, time to first stool, overall complications, as well as long term results between two groups. Conclusions: Intracorporeal anastomosis in laparoscopic colectomy is safe, feasible, and does not increase the overall complications and the overall operative time. In addition, this technique also help patients recover faster by significantly reducing the time to first flatus, the postoperative hospital length of stay, as compared to extracorporeal anastomosis.
