SURGICAL FEATURES AND OUTCOME OF INVERTED KIDNEY TRANSPLANTATION AT VIETDUC HOSPITAL FROM 2020 TO 2023

Abstract
Objective: To describe the technical characteristics, indications and evaluate the results of inverted kidney transplantation. Subjects and methods: Retrospective descriptive study of cases of inverted kidney transplantation at Viet Duc University Hospital from January 2020 to December 2023. Results: There were 11 patients who underwent inverted kidney transplantation, all transplanted with right kidney graft into the right iliac fossa. The mean age 48.9 ± 11.7 year old, male / female ratio 1.75/1. 1 graft renal artery 72.7% and 1 graft renal vein 90.9%. Indications for inverted kidney transplant are to avoid severe atherosclerosis in the upper half of the external iliac artery in 6 patients, accounting for 54.5%, to avoid angulation due to early branching and long renal artery in 5 patients, accounting for 45.5%, no cases of inverted kidney transplantation due to short renal vein. Most of cases Creatinine returns to normal on day 7 after transplantation. No early medical or surgical complications after kidney transplantation, about the late results, all 11 patients had no vascular complications like pseudoaneurysm, thrombosis or vascular kinking and angulation. No complication of ureteral stenosis, no complications of ureteral-bladder anastomosis, no acute rejection, and no CMV or BK infection after transplantation. Conclusion: Inverted kidney transplantation is a useful and safe technique to manage the atherosclerosis in the upper half of the external iliac artery or early branching and long graft renal artery to avoid angulation.
