RECURRENT SYMPHATHETIC OPTHALMIA: A CASE REPORT

Abstract
Sympathetic ophthalmia (SO) is a rare, severe bilateral diffuse granulomatous panuveitis that usually results from surgical or penetrating trauma to one eye. The almost all cases of SO develop within 3 months after penetrating trauma and eye surgery. We report a case of a 19-year-old male with sudden decrease of visual acuity in the right eye, 50 days after penetrating injury to the left eye. The patient was treated with corticosteroids monotherapy, but after 2 months, blurred vision recurred. He has been diagnosed with relapsing SO, so we decided to combine treatment with pulse methylprednisolone 1000mg/day x 3 days and cyclosporin A 150mg/day. After 10 months of treatment, visual acuity should improve (20/40), and inflammatory index tests were normal. In conclusion, a combination immunosuppressive therapy with close monitoring is crucial in the management of sympathetic ophthalmia to achieve remission, prevent recurrence, and potentially avoid the need for surgical intervention.
