ASSESSMENT OF PERIPAPILLARY VASCULAR DENSITY USING OCT-A AFTER TRABECULECTOMY IN PRIMARY OPEN-ANGLE GLAUCOMA

Abstract
Purpose: To investigate peripapillary vascular density (VD) before and after trabeculectomy in patients with primary open-angle glaucoma (POAG). Methods: This prospective, interventional, comparative study included 36 eyes of 36 patients with POAG who were followed for 6 months postoperatively. Peripapillary VD was measured using optical coherence tomography angiography (OCT-A) with Angioplex software. Intraocular pressure (IOP) was also measured preoperatively and at 1, 3, and 6 months postoperatively. Paired comparisons were performed between preoperative and postoperative values. Results: Thirty patients (83.3%) had advanced glaucoma and 6 patients (16.7%) had moderate glaucoma. The mean preoperative peripapillary VD was 38.79 ± 2.83%. There was a statistically significant decrease in VD at 6 months postoperatively (p<0.05). The mean preoperative IOP was 31.33 ± 7.08 mmHg. Postoperatively, IOP was reduced by a mean of 59.37% from baseline, and there was a statistically significant decrease in IOP at all postoperative time points (p<0.05). Conclusion: Peripapillary VD may continue to decrease in patients with POAG despite IOP reduction achieved with trabeculectomy.
