EFFICIENCY AND SAFETY OF INDUCED LABOR BY PROSTAGLANDIN E2 IN OVERDUE PREDNANCY IN GIA DINH PEOPLE’S HOSPITAL

Abstract
Background: An initiating labor before spontaneous onset of labor in a viable pregnancy, called as induction of labor (IOL), artificially initiates the process of effacement of the cervix, dilatation of the cervix. With advancements in medicine, various methods of labor induction have been implemented. These methods are categorized into two main groups: mechanical and pharmacological. Mechanical methods include: amniotomy, cervical dilation using Laminaria, Kovac’s dilator, single-balloon Foley catheter, and double-balloon Foley catheter. Pharmacological methods include: Oxytocin, Prostaglandin E1 (Misoprostol), and Prostaglandin E2 (Dinoprostone). In this study, we focus on labor induction using Prostaglandin E2 (Dinoprostone). Objective: To determine the success rate of IOL using Dinoprostone in women beyond the 40 weeks of pregnancy at Gia Dinh People’s Hospital. Method: A retrospective case series was conducted between April 2021 and September 2021, enrolling 46 women beyond the 40 weeks of single pregnancy who underwent Dinoprostone slow-released vaginal insert-induced labor at Gia Dinh People’s Hospital.The primary outcome was defined as the Bishop score ≥ 7 within 24 hours induction with Dinoprostone vaginal insert. The secondary outcome analyzed perinatal complications, adverse effects. Result: Among 46 women who underwent Dinoprostone slow-released vaginal insert-induced labor, the success rate of IOL using Dinoprostone was 89.13% (CI 84.67-93.58). The rate of vaginal delivery was higher than cesarean delivery (65.21% CI 51.5-78.9vs 34.78% CI 21.1 - 48.5)). We assigned some side effects: nausea (4.34%), diarrhea (4.34%) and some perinatal complications: fetal distress (8.69%), uterine hyperstimulation (17.39%), rupture membrane (2.17%), hemorrhage postpartum (4.34%), apgar at 5 minutes < 7 (4.34%). Conclusion: Dinoprostone slow-released vaginal insert is a good choice for women beyond the 40th week of single pregnancy. Because of some perinatal complications, we need comply the process.
