Abstract

Objective: Evaluation of the effectiveness of gluteus maximus flap combined with negative pressure suction in the treatment of chronic sacral ulcers. Methods: Observational study, cohort analysis of 30 patients with chronic sacral ulcers, treated at Hospital 199, Ministry of Public Security from January 2022 to September 2024. Results: The rate of male is 63%, the most common age group > 60 years old accounts for 52%. The main causes of ulcers are immobility due to old age (30%), cerebrovascular accident (TBMMN) (36%), spinal cord injury (27%). The state of infection and necrosis after debridement and combined treatment with negative pressure suction therapy (V.A.C) is 0% with 100% of the wound base on pink granulation tissue, creating conditions for surgery to transfer the gluteus maximus muscle skin flap, the result of complete survival of the flap after surgery is 25/30 (86.33%). Conclusion: Debridement + V.A.C prepares the ground to facilitate plastic surgery to transfer flaps to cover the defect. The patients are mainly elderly, with sequelae of spinal cord injury or localized TBMMN, so they are prone to recurrent ulcers and also have more difficulty in post-operative flap care.
