Abstract

Introduction: Managing non-communicable diseases (NCDs) such as hypertension (HTN) and diabetes mellitus (DM) is a major challenge, especially in mountainous areas. This study evaluates the effectiveness of the family medicine model in managing HTN and DM in Cao Bang Province. Methods: The study was conducted at three commune health stations from January to May 2024, using a community intervention approach with pre- and post-intervention evaluations. Results: After the intervention, the percentage of patients aware of their blood pressure and blood glucose levels increased to 100% and 90.9%, respectively. The rates of continuous follow-up and achieving target blood pressure significantly improved (99.5% and 61.6%, respectively). The percentage of diabetic patients achieving target blood glucose increased from 22.7% to 63.6%, with p < 0.001. Conclusion: The family medicine model has enhanced the management of HTN and DM patients in Cao Bằng.
