DIAGNOSTIC VALUE OF 1.5 TESLA MRI IN PREDICTING METASTATIC AXILLARY LYMPH NODES IN BREAST CONSERVING SURGERY CANDIDATES

Abstract

Purposes: To evaluate the diagnostic value of breast MRI to predict metastatic axillary lymph nodes in breast conserving surgery candidates. Materials and methods: 76 axillae in 76 patients were studied. The size and morphological parameters on T2-weighted images and apparent diffusion coefficient (ADC) values were analyzed. An independent t-test/chi-square test and receiver operating characteristics (ROC) curve analysis were used. Results: Short axis length, short axis/long axis ratio, cortical thickness, loss of fatty hilum and eccentric cortical thickening were statistically significantly different between the metastatic and nonmetastatic groups. The long axis length was not a statistically significant parameter. The ADC value was significantly different between the 2 groups, with an AUC that was higher than that of conventional MRI parameters (AUC: 0.874; threshold < 1.046 x 10-3 mm2/s ; sensitivity: 78.6%; specificity: 85.5%). Conclusion: Using the combination of conventional MRI and DWI, we can predict metastatic lymph nodes in breast conserving candidates with high sensitivity and high specificity.
