Abstract

Objective: To evaluate the feasibility and safety of laparoscopic liver resection (LLR) for treating hepatocellular carcinoma (HCC) in segment 4 of the liver. Subjects and Methods: From October 2021 to October 2024, a total of 44 patients with HCC in segment 4 underwent LLR at the University Medical Center in Ho Chi Minh City. We conducted a retrospective study describing the clinical characteristics and early postoperative outcomes. Results: The male-to-female ratio was 88.6%, with a mean age of 58 years (range: 14–72). Tumors were located near major hepatic vessels in 18 cases (40.9%). Left hepatectomy and segment 4 liver resection accounted for 61.4% and 38.6% of cases, respectively. The mean operative time was 194 minutes (range: 100–360), and the mean blood loss was 162 mL (range: 30–500 mL). No severe complications or postoperative mortality were recorded. The median hospital stay was 6 days (range: 4–12). All cases achieved negative surgical margins. Conclusion: LLR for HCC in segment 4 of the liver is feasible and safe, even in cases where tumors are located near major hepatic vessels.
