Abstract

Objective: To investigate the rate of beta-blocker up-titration and associated factors related to the lack of dose escalation in patients with heart failure with reduced ejection fraction (HFrEF). Subjects and Methods: A cross-sectional descriptive study was conducted at the Outpatient Department of An Giang Cardiovascular Hospital from April to September 2024. The study population included HFrEF patients receiving beta-blockers with complete follow-up visits. Results: Among 251 patients, the rates of dose escalation at the first and second follow-up visits were 13.1% and 13.9%, respectively. The main reasons for not increasing the dose were bradycardia (49.1% and 50.9%), hypotension (15.1% and 13%), and physician hesitation (35.3% and 35.6%). No statistically significant associations were found between sex, age, NYHA class, coronary artery disease, or physician experience and the lack of up-titration. Conclusion: The rate of beta-blocker up-titration in HFrEF patients remains low. Interventions are needed to improve the achievement of optimal target doses in heart failure management.
