THE EFFECT OF TRIANGULAR AND ENVELOPE FLAP DESIGNS TO CLASS II PARANT LOWER THIRD MOLAR EXTRACTION AT TU SON CITY MEDICAL CENTER IN 2023

Abstract
Objective: To describe the post-operative results between two groups using envelope and triangular flap in class II Parant wisdom tooth extraction at Tu Son City Medical Center. Subjects: Patient Parant II classification wisdom tooth extraction from February 2023 to June 2023. Methods: Clinical intervention without control. Results: 42 patients with envelope flap and 40 patients with triangular flap had lower wisdom teeth removed. The average VAS scores when using envelope flap at 6 and 24 hours were 3.33 ± 1.18 and 0.79 ± 0.78, respectively. The average VAS scores when using the triangular flap at 6 and 24 hours were 3.93 ± 1.69 and 1.15 ± 1.21, respectively. The swelling index on the 2nd day post-surgery in the group using the envelope flap was 111.35 ± 4.6 mm, while the group using the triangular flap had a value of 108.56 ± 5.6mm. The mouth opening range at 7 days post-operative was 50.1 ± 3.3 mm for the envelope flap group and 50.6 ± 2.8 mm for the triangular flap group. There were no cases of prolonged bleeding post-surgery in the envelope flap group, while in the triangular flap group, bleeding after 6 hours was 10%, and after 24 hours, it was 5%. 71,43% of patients in the envelope flap group had secondary wound healing, and 75% of patients in the triangular flap group had primary wound healing. Conclusion: The average VAS score at 6 hours and 24 hours was lower in the envelope flap group compared to the triangular flap group. The swelling index on the 2nd day post-surgery was lower in the envelope flap group compared to the triangular flap group. There were no significant differences in post-surgical bleeding levels or complications in tooth extraction between the two groups. The rate of primary wound healing was significantly higher in the triangular flap group compared to the envelope flap group.
