EVALUATION OF OOCYTE AND EMBRYO MORPHOLOGY IN IVF PATIENTS USING AN ANTAGONIST PROTOCOL WITH DELTA-FOLLITROPIN (REKOVELLE)

Abstract
Objective: To evaluate the morphology of oocytes and embryos in patients undergoing ovarian stimulation with the antagonist protocol using Delta-follitropin (Rekovelle). Subjects and Methods: A combined retrospective and prospective cross-sectional descriptive study conducted on 71 infertile women treated at the Military Clinical Embryology Institute from August 2023 to December 2024. Results: The average age of patients was 36.31 ± 4.58 years; all had previously undergone IVF, with 31% having had two or more cycles. Most patients had a normal BMI (81.7%) and AMH levels below 2.1 ng/ml (71.8%). The average stimulation duration was 10.14 ± 1.427 days, with a mean total Rekovelle dose of 176.28 ± 35.98 µg. There was a positive linear correlation between E2 levels on trigger day and the number of follicles ≥14 mm. The average number of retrieved oocytes was 5.41 ± 3.52; mature oocytes (MII) averaged 4.3 ± 3.11; and the mean fertilization rate was 74.44 ± 28.84%. There was a statistically significant difference in the number of oocytes retrieved between age and AMH groups. The average number of blastocysts on days 5–6 was 1.92 ± 2.36, with 0.77 ± 1.38 being of good quality. Most blastocysts reached expansion grade 3 (83.82%). A significant association was observed between the morphology of the inner cell mass and the trophectoderm. Conclusion: This study successfully evaluated oocyte and embryo morphology in patients using the antagonist protocol with Delta-follitropin (Rekovelle), providing a foundation for future research and assessment in infertility treatment protocols.
