Abstract

Purpose: To evaluate and describe the multidetector computed tomography (CT) features of gastric lymphoma in patients with histopathologically confirmed diagnosis. Materials and Methods: In this prospective cross-sectional study, 45 patients with pathologically proven gastric lymphoma underwent contrast-enhanced CT at 108 Military Central Hospital and E Hospital from October 2015 to January 2025. All patients had prior endoscopic biopsy confirmation. CT parameters assessed included tumor location, size, enhancement characteristics, and associated lymphadenopathy. Data analysis was performed using SPSS version 22.0, and interobserver agreement was calculated using the Kappa statistic. Results: The mean patient age was 57.42 ± 9.75 years; the male-to-female ratio was 3.5:1. The most common tumor location was the distal third of the stomach. Mean tumor dimensions were 72.9 ± 27.2 mm in length, 30.3 ± 16.6 mm in width, and 18.4 ± 8 mm in thickness. The average enhancement of the tumor was 68.22 ± 7.80 HU. A total of 398 lymph nodes were detected, predominantly regional (90.41%), with 44.2% measuring <6 mm. Most lymph nodes were oval (75.1%) and demonstrated marked enhancement (87.2%). The average enhancement of the lymph nodes was 56 ± 14,49 (20,12 – 72,12) HU. Conclusion: Gastric lymphoma typically appears as a moderately enhancing distal gastric mass with enhancing regional lymphadenopathy, showing lower enhancement than gastric adenocarcinoma.
