VALUE OF H3B2 SCORE IN PROGNOSIS OF NON-VARICEAL UPPER GASTROINTESTINAL BLEEDING

Abstract

Objective: To evaluate the value of the H3B2 score in predicting rebleeding and in-hospital mortality in patients with non-variceal upper gastrointestinal bleeding (GIB). Subjects and methods: A prospective, cross-sectional study on 254 patients with GIB not due to portal hypertension at the Department of Gastroenterology - Military Hospital 103. Calculate the H3B2 score and compare it with the rate of rebleeding and mortality during the patient’s hospital stay. Results: The H3B2 score has good value in predicting the risk of in-hospital rebleeding (AUC = 0.83; 95%CI: 0.73-0.93; p < 0.001), at the cut-off point of 4.5, the sensitivity was 78.6%, the specificity was 81.7% and the negative predictive value was 98.4%. In predicting in-hospital mortality, the H3B2 score only achieved a moderate value with AUC = 0.71; 95%CI: 0.60-0.82; p = 0.002. At the same cut-off point of 4.5, the sensitivity was 55.0%, the specificity was 81.2% and the negative predictive value was 95.4%. Conclusion: H3B2 is a score with good value in prognosis of non-variceal upper gastrointestinal bleeding.
