RESEARCH THE COMPUTED TOMOGRAPHY IMAGING OF POSTAURICULAR EXTRACRANIAL ACUTE MASTOIDITIS IN PATIENTS AT THE VIETNAM NATIONAL CHILDREN'S HOSPITAL

Abstract
Objectives: The aim of this study is to describe the computed tomography (CT) imaging characteristics of postauricular extracranial acute mastoiditis in patients treated at the Vietnam National Children's Hospital. Acute mastoiditis is a bacterial infection of the mastoid air cell system surrounding the middle ear. The inflammatory process is considered acute if it lasts less than 3 months. Approximately 20% of acute otitis media cases may progress to acute mastoiditis that causes extracranial complications, including postauricular extracrania. Subjects and Methods: Cross-sectional descriptive study involving 23 pediatric patients that diagnosed acute mastoiditis with Postauricular Extracranial Complications, performed CT imaging at the Vietnam National Children's Hospital from August 2023 to March 2024. Results and Discussions: Males accounted for 56.52%, females accounted for 43.48%, ages 1-3 were most common at 43.48%; Mastoiditis in both ears was higher at 52.17%; CT scan images of mastoid sinus opacity accounted for 100%, periosteum thickening 100%, Bony necrosis was identified in 82.61%, extracranial lesions 78.26%; The level of mastoid bone damage on CT shows periosteum bulging 91.30%, 82.61% cell wall disruption, 78.26% mastoid cortex disruption. Conclusions: Acute mastoiditis with postauricular extracranial mastoiditis occured most often in the age group of 1 - 3 years old, 100% acute mastoiditis patients with postauricular extracranial complications on CT scan imaging show mastoid bone opacity and periosteum thickening, in which the rate of mastoid cortex destruction is relatively high at 78.26%.
