Abstract

Objective: To determine the value of the plasma CRP/Albumin ratio in predicting the severity of acute pancreatitis (AP). Subjects and methods: A prospective, cross-sectional study on 142 AP patients treated at the Department of Gastroenterology and the Emergency and Resuscitation Center - Military Hospital 103. CRP and Albumin were measured on plasma at the time of admission and 24-48 hours after admission, compared with the severity of AP according to the revised Atlanta classification 2012. Results: There was no difference in the concentrations of Albumin, CRP and the CRP/Albumin ratio on plasma at the time of admission between the patient groups according to the severity of AP. However, at time T1 (24-48 hours after admission), the Albumin concentration decreased significantly, while CRP and the CRP/Albumin ratio on plasma increased significantly compared to T0 (time of admission) (p < 0.001). The CRP/Albumin ratio at 24 - 48 hours had a good predictive value for severe AP with an AUC of 0.81, at a cut-off point of 10.27 with a sensitivity of 54.5% and a specificity of 93.1%. Conclusion: The CRP/Albumin ratio is a simple non-invasive index useful in the prognosis of severe AP.
