THE ASSOCIATION BETWEEN LEFT ATRIAL STRAIN AND STIFFNESS WITH LEFT ATRIAL MORPHOLOGY AND FUNCTION ON ECHOCARDIOGRAPHY IN PATIENTS WITH END-STAGE CHRONIC KIDNEY DISEASE

Abstract

Objective: To investigate the association between left atrial (LA) strain and stiffness with LA morphology and function in patients with end-stage chronic kidney disease (ESKD). Subjects and Methods: A cross-sectional descriptive study was conducted on two groups of participants. The patient group included 62 individuals with end-stage chronic kidney disease treated at 103 Military Hospital from October 2024 to April 2025. The control group consisted of 42 healthy individuals matched for age and gender. Results: Reservoir strain (LASr) measured in four-chamber (4C) and two-chamber (2C) views showed a moderate positive correlation with total left atrial emptying fraction (LATEF) and passive left atrial emptying fraction (LAPEF). Conduit strain (LAScd) in 4C and 2C views demonstrated a moderate positive correlation with pLAEF and a moderate negative correlation with the active left atrial emptying fraction (LAAEF) and left atrial diameter index (LADi). Contractile strain (LASct) in 2C and 4C views had a moderate positive correlation with LAAEF and a weak positive correlation with LATEF. LA stiffness index (LASI) was moderately negatively correlated with LAPEF and weakly positively correlated with LAAEF and LADi. Conclusion: Left atrial reservoir strain (LASr) is associated with LATEF and LAPEF; conduit strain (LAScd) is associated with LADi, LAAEF, and LAPEF; contractile strain (LASct) correlates with LAAEF and LATEF; and LA stiffness index (LASI) is associated with LADi, LAAEF, and LAPEF.
