DOULEUR NEUROPATHIQUE 4 (DN4) SCALE FOR NEUROPATHIC PAIN IN PATIENTS WITH LUMBAR SPINE OSTEOARTHRITS

Abstract

Objective: Description of clinical, radiological and pain characteristics due to neuropathic causes according to the DN4 scale in patients with lumbar spine osteoarthritis. Subjects: Conducted a study on 105 patients diagnosed with lumbar spine osteoarthrits who had VAS spinal pain score ≥ 4 points and were treated in the Centre for Rheumatology from August 2024 to August 2025. Research Method: Cross-sectional study. Result: The average age of the study subjects was 65,8 ± 11.5 years; the majority were female (69,1%);  (58,1%). Lumbar Mainly manual labor (91.4%) spinal pain primarily occurred spontaneously (81,5%); pain duration is mainly >6 months (50,0%). Most common clinical symptoms: pain on palpation of paraspinal pain points (100%), paraspinal muscle stiffness (85.2%), decreased Schober index (72,8%), increased hand-to- ground distance (70,4%). 2. Most common X-ray symptoms: subchondral sclerosis (72.8%), vertebral body osteophytes (98.8%). 3. The rate of neuropathic pain in patients with degenerative lumbar spine disease according to the DN4 scale was 30,7%. Common neuropathic pain symptoms according to the DN4 scale were painful cold (84,8%), electric shocks (69,7%),  (61,9%) and numbness (90.9%). Conclusion: The rate of lumbar spine pain due to neuropathic pain in patients with degenerative lumbar spine disease according to the DN4 scale is 30,7% and neuropathic pain may be 30,7%. Common symptoms of neuropathic pain according to the DN4 scale are numbness, painful cold,electric shocks.
