Abstract

Objectives: To describe the characteristics and evaluate treatment outcomes of out-of-hospital cardiac arrest (OHCA) patients witnessed by bystanders at the A9 Emergency Department – Bach Mai Hospital. Methods: A prospective descriptive study was conducted on 121 OHCA patients admitted between June 2024 and June 2025. Inclusion criteria were: cardiac arrest occurring outside a medical facility, witnessed by a bystander, patient aged ≥18 years, and cardiac arrest confirmed by emergency medical staff. Data were collected from medical records and interviews with bystanders. Neurological outcomes at discharge were assessed using the Cerebral Performance Categories (CPC) Scale. Results: Male patients accounted for 70.2%, with a mean age of 57.6 ± 17.3 years. CPR was performed by bystanders in only 22.1% of cases, mostly by family members or healthcare providers. Patients who received bystander CPR had significantly higher rates of pre-hospital ROSC and survival to discharge compared to those who did not. Conclusions: Early bystander CPR was strongly associated with improved ROSC and survival outcomes in OHCA patients. The proportion of patients receiving cardiopulmonary resuscitation (CPR) from bystanders remains relatively low. Promoting community CPR training and strengthening prehospital emergency response systems are crucial to improving OHCA management in Vietnam.
