Abstract

Background: In 2024, measles outbreaks have resurged globally and in Vietnam, with predictions indicating continued complexity into 2025. At Da Nang Obstetrics and Pediatrics Hospital, the number of pediatric admissions due to measles increased from late 2024 to early 2025. However, no published studies to date have investigated factors associated with the severity of pneumonia complications in children with measles. Objective: To identify factors associated with the severity of pneumonia complications in pediatric measles patients. Subjects and Methods: A cross-sectional study was conducted on children aged 1 month to 15 years diagnosed with measles and treated at Da Nang Obstetrics and Pediatrics Hospital. Measles diagnosis was based on the 2014 and 2025 Ministry of Health guidelines, which include clinical or suspected cases with positive measles-specific IgM antibodies. Pneumonia diagnosis and classification were performed according to the 2014 Ministry of Health criteria. Results and Discussion: From February to May 2025, 243 children with measles and pneumonia complications met the inclusion criteria. Among them, 74 cases (30.5%) had severe pneumonia. Five factors were significantly associated with severe pneumonia complications: underlying medical conditions, tachypnea, chest indrawing, anemia, and elevated C-reactive protein (CRP) (p < 0.05). Multivariate logistic regression identified three independent risk factors: underlying conditions, tachypnea, and chest indrawing (p < 0.05). Conclusion: Underlying diseases, tachypnea, chest indrawing, anemia, and elevated CRP levels were found to be significantly associated with the severity of pneumonia complications in children with measles.
