BURDEN OF DEPRESSION IN OLDER OUTPATIENTS HAVING HEART FAILURE WITH REDUCED EJECTION FRACTION

Abstract
Background: Older patients with heart failure often experience geriatric issues, including depressive symptoms. Depression affects the quality of life and prognosis in older patients with heart failure. Therefore, identifying the presence of depression is essential in clinical practice for the care and treatment of older patients with heart failure. Objective: To investigate the prevalence and factors associated with depression in older outpatient having heart failure with reduced ejection fraction (HFrEF). Methods: Cross-sectional study. The sample population included older outpatient patients with HFrEF at Thong Nhat Hospital and University of Medicine and Pharmacy Hospital in Ho Chi Minh City from August 2024 to January 2025. Depression was assessed using the Geriatric Depression Scale-15 (GDS-15). Factors associated with depression were identified through logistic regression. Results: Among the 218 patients included in the study (mean age: 72.5 ± 8.9; male: 59.2%), 106 patients (48.6%) had depression. Factors associated with depression included single/divorced/widowed (OR 5.29; 95% Confidence Interval [CI] 1.01–27.6; P = 0.049), dependent IADL (OR 3.75; CI 95% 1.48–9.48; P = 0.005). and NYHA class III (OR 4.26; CI 95% 1.52–11.9; P = 0.006). Conclusion: In older outpatient with HFrEF, prevalence of depression was 48.6%. Single/divorced/ widowed, dependent IADL, and NYHA Class III are factors associated with depression.
