INITIAL RESULTS OF APPLYING 3D PRINTING TECHNOLOGY IN GAMMA 3 NAIL SURGERY FOR INTERTROCHANTERIC FEMUR FRACTURES IN ELDERLY PATIENTS

Abstract
Objective: To apply 3D printing technology in the surgical treatment of intertrochanteric femoral fractures using Gamma 3 nails in patients aged 60 years or older with osteoporosis. Materials and Methods: A prospective case series study of 15 patients aged ≥60 years with unstable closed intertrochanteric femoral fractures due to trauma, who underwent Gamma 3 nail fixation with 3D printing assistance at the Lower Limb Department, Emergency Department, Ho Chi Minh City Orthopedic Trauma Hospital from 7/2024 - 12/2024. Results: The median age was 77 years (IQR: 72–81), with females accounting for 73.3%. The main cause was domestic accidents (86.7%); left-sided fractures accounted for 46.7%, right-sided 53.3%; AO 31A2 accounted for 80%. Surgical characteristics: ASA III in 53.3%; median operative time 50 minutes, with 53.3% < 60 minutes; median blood loss 200 ml, with 80% ≤ 200 ml. All cervical–head screws were placed accurately; limb shortening occurred in 16.7%; no major complications recorded. VAS pain score decreased from 6.5 postoperatively to 1.8 at 3 months; Harris Hip Score improved from 72.9 (1 month) to 79.1 (3 months). Conclusion: The initial application of 3D printing technology in Gamma 3 nail surgery for intertrochanteric femoral fractures in elderly patients appears feasible, safe, and has the potential to improve surgical outcomes.
