BLOCK FAT GRAFTING FOR THE CORRECTION OF SUNKEN UPPER EYELIDS: A CLINICAL CASE SERIES

Abstract

Sunken upper eyelid is a common feature among Asian individuals, which may be attributed to genetic predisposition, congenital anatomy, or age - related changes. It frequently appears in middle - aged adults due to orbital fat atrophy, skin laxity, skeletal remodeling, or as a consequence of previous surgeries - including aesthetic procedures such as upper blepharoplasty, ptosis correction, or brow lift surgery - in which excessive orbital fat may have been removed. In East Asian aesthetics, the eyes are considered the “windows to the soul,” reflecting both inner beauty and personal expression. An ideal upper eyelid is not only defined by symmetrical double folds, but also by fullness and softness in the upper lid area -features symbolizing youthfulness, refinement, and charm. Therefore, despite not impairing ocular function, sunken upper eyelids have a considerable impact on facial aesthetics and patients' psychological well-being. Autologous fat grafting has been recognized as an effective treatment for this condition. Among available techniques, block fat grafting - which preserves the vascular architecture of the graft - offers long - lasting outcomes and lower fat resorption rates compared to centrifuged fat. This method is highly effective, technically straightforward, easy to perform, and cost - efficient, making it an increasingly preferred option for correcting sunken upper eyelids.
