EARLY PAIN-RELIEF OUTCOMES OF CRYOTHERAPY COMBINED WITH PATIENT-CONTROLLED ANALGESIA IN POSTOPERATIVE CARE FOR TOTAL HIP ARTHROPLASTY PATIENTS

Abstract
Objectives: To evaluate pain intensity, and total fentanyl consumption within 24 hours among patients undergoing THA treated with cryotherapy combined with intravenous PCA. Methods: A prospective longitudinal descriptive study was conducted on 48 patients undergoing elective total hip arthroplasty at Cho Ray Hospital between August 2024 and June 2025. Data were collected from medical records and clinical assessments, including: pain scores measured by the Visual Analog Scale (VAS) and total fentanyl dose administered via PCA in the first 24 hours postoperatively. Data were entered using Epidata 3.1 and analyzed with STATA version 14.0. Descriptive statistics included frequencies, percentages, medians, and interquartile ranges (IQR). Results: On postoperative day 3, 87,5% of patients reported mild pain, while 12,5% experienced moderate pain. The mean total fentanyl consumption over 24 hours was 178,35 ± 44,45 mcg. Conclusion: The pain level of patients who received cold therapy in combination with patient-controlled analgesia after total hip arthroplasty remained within the mild to moderate range during the first three postoperative days. This combination is a promising approach that brings effectiveness in the postoperative care of patients undergoing hip replacement.
