SURGICAL OUTCOMES OF THUMB REPLANTATION AFTER TRAUMATIC AMPUTATION AT 108 MILITARY CENTRAL HOSPITAL

Abstract
Objective: To describe the clinical and radiographic characteristics of thumb amputations and evaluate the outcomes of thumb replantation using microsurgical techniques. Subjects and Methods: A cross-sectional descriptive study combining retrospective and prospective data on 30 patients with thumb amputation who underwent replantation at the 108 Military Central Hospital (from June 2020 to June 2025). Results: In thumb replantation after amputation, the survival rate was 93,33% (28/30). The incomplete amputation group had the best outcome with 100% survival (16/16), while the complete amputation group reached 85,7% (12/14). Sharp injuries and those in zones II–IV had favorable outcomes (91,7–100%), whereas injuries in zone III showed less favorable results. At 3-month follow-up, 95% of patients achieved grade I or II functional outcomes, with substantial sensory and motor recovery according to the Chung Wei Chen criteria. Conclusion: Thumb replantation after amputation demonstrated a high survival rate of 93,3% (28/30). Factors influencing survival outcomes included near-complete amputation, sharp injuries, ischemia time under 6 hours, and proper preservation. Long-term evaluation showed that 95% of patients achieved grade I or II outcomes, confirming the effectiveness and feasibility of microsurgical thumb replantation.
