Abstract

Objective: To study the clinical and paraclinical characteristics, evaluate the complications of tonsillectomy in adults over age 45. Methods & Materials: A prospective, descriptive study on 60 patients who required tonsillectomy and operated at National Otorhinolaryngology Hospital. Results: The average age was 53,33±7,48, the oldest was 80 years old, female/male ratio = 1,32/1, the average BMI was 22,77±2,78. The main indications were recurrent tonsillitis, enlarged tonsils that cause upper airway obstruction, requiring biopsy to define tissue pathology and tonsil cancer. With taking only tonsillectomy group: the mean surgical duration was 21,5± 3,9 minutes, 85,4% cases the amount of blood loss during the operaton less than 5ml. And on group that taking tonsilectomy combine with others: Surgical duration and blood loss during the operation were higher. Postoperatve bleeding was 8,34% (5 of 60) of the cases. 80% bleedings were mild. Conclusion: The study showed that age is not a contraindication to tonsillectomy. The main indications for tonsillectomy in patients over age 45 were included: Recurrent tonsillitis is still the most popular while enlarged tonsils that cause upper airway obstruction, tumors of tonsil were more common. It is obvious that tonsillectomy in adults have some difficulties due to the fibrosis that leads to find the tonsil boundary hardly and comorbidities, which raises the rate of postoperatve bleeding. However most of the bleedings were mild and unnecessary to blood transfusion or taking sugery to stop bleeding. Pillar suture might reduce the risk of postoperation bleeding.
