MEAN PLATELET VOLUME IN PATIENTS WITH ACUTE EXACERBATIONS OF CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Abstract
Objective: To assess the utility of mean platelet volume (MPV) in identifying acute exacerbations of COPD (AECOPD) and predicting severity. Methods: Prospective cross-sectional study of 412 COPD patients (inpatients = 128; outpatients = 284), including 177 AECOPD and 235 stable cases (Aug-2023 to Sep-2024, Bach Mai Hospital). Pulmonary function, hematology/biochemistry, platelet indices and MPV were collected and analyzed (SPSS 20; ROC for MPV cut-off). Results: Mean age 71 ± 9 years; male predominance. Overall FEV₁(%) 43.45 ± 16.36 with non-significant differences between AECOPD and stable groups. MPV cut-off 7.95 fL, AUC = 0.94 (p<0.001), sensitivity = 84%, specificity = 84% for distinguishing AECOPD from stable COPD. MPV rose from day 1→4→7; MPV ≥ 7.95 associated with higher Anthonisen severity (Types 2, 3). Conclusions: MPV is a practical marker to support AECOPD identification and severity stratification; the 7.95 fL cut-off shows strong diagnostic performance.
