Abstract

Metaplastic breast carcinoma (MBC) is a rare histological subtype, accounting for less than 1% of invasive breast cancers, characterized by the coexistence of epithelial and mesenchymal components. We report the case of a 33-year-old female who presented with a rapidly enlarging right breast mass; ultrasound and MRI revealed a heterogeneous lesion with central necrosis and rim enhancement. Core needle biopsy confirmed MBC, triple-negative with a high Ki-67 index (60%). The patient underwent total mastectomy with negative sentinel nodes (pT3N0) and received adjuvant chemotherapy, but developed early local recurrence during treatment. This case illustrates the typical clinical and imaging features of MBC, while highlighting its poor prognosis and limited response to chemotherapy, emphasizing the importance of accurate diagnosis, close follow-up after surgery, and consideration of novel systemic treatment strategies.
