RESULTS OF ANTERIOR CRUCIATE LIGAMENT RECONSTRUCTION WITH THE ALL-INSIDE TECHNIQUE USING THE SEMITENDINOSUS TENDON AT KHANH HOA GENERAL HOSPITAL

Abstract

Objective: To determine the morphology of the semitendinosus tendon graft and to evaluate the results of arthroscopic anterior cruciate ligament (ACL) reconstruction surgery with the All-inside technique using the semitendinosus tendon. Subjects and methods:  A prospective study was conducted on 36 patients aged 18 to 45 with ACL rupture who underwent arthroscopic ACL reconstruction using the All-inside technique using a  semitendinosus tendon at Khanh Hoa General Hospital from January 1, 2024 to December 31, 2024 and a follow-up period of at least 9 months. Results: The semitendinosus tendon demonstrated a mean length of 26.8 ± 1.5 cm (24 – 29 cm). Following quadrupling and whipstitching, the resulting graft's mean diameter and mean length were 7.8 ± 0.5 mm (7 – 9 mm), 61.2 ± 1.2 cm (59 – 64 cm), respectively. At the 9-month post-operative follow-up, clinical testing indicated a significant improvement in knee stability: the Anterior Drawer Test was negative in 77.8% of patients, the Lachman Test was negative in 80.6%, and the Pivot Shift Test was negative in 100%. Functional outcomes, measured by the Lysholm Knee Score at 9 months, showed 91.7% of patients achieving results in the Excellent and Good categories. Furthermore, 83.3% of patients had successfully returned to their pre-injury Tegner activity level. Conclusions: The arthroscopic ACL reconstruction method with the All-inside technique using the semitendinosus tendon can effectively ensure good graft size while causing less damage to the graft donor site. This technique helps to reconstruct the ligament smoothly, ensuring a stable graft and results in less bone tunnel damage at the recipient site due to the use of shorter tunnel drilling compared to traditional methods. Additionally, postoperative results indicate that the majority of patients can return to their pre-injury work and physical activity levels.
