CLINICAL, BIOMAKERS CHARACTERISTICS AND OUTCOMES OF PATIENTS HOSPITALIZED WITH ACUTE EXACERBATIONS OF CHRONIC OBSTRUCTIVE PULMONARY DISEASE AT DISTRICT 7 HOSPITAL

Abstract

Objective: To investigate clinical and biomakers characteristics and identify factors associated with poor clinical outcomes in patients hospitalized due to acute exacerbations of chronic obstructive pulmonary disease (AECOPD). Methods: A prospective cohort, non-interventional study was conducted on 112 patients admitted with AECOPD to Nguyen Thi Thap General Hospital in Ho Chi Minh City from April 2023 to August 2024. Demographic, clinical and biomakers  data were collected within the first 24 hours of hospitalization. Results: The majority of patients were male (88.4%) and aged ≥60 years (73.2%). Poor outcomes - defined as requiring mechanical ventilation or resulting in death - occurred in 7.1% of cases. Significant factors associated with poor outcomes included older age (75.6 ± 10.7 vs. 66.7 ± 9.8 years; p = 0.015), presence of respiratory failure (OR = 32.2; p < 0.0001), higher CAT scores (27.0 ± 8.9; p = 0.036), lower PaO₂, elevated PaCO₂, reduced HCO₃⁻ levels (all p < 0.01), and low FEV1 (p < 0,05),  Patients requiring noninvasive and invasive mechanical ventilation interventions are strongly associated with poorer clinical outcomes (p<0,01). No significant associations were observed between poor outcomes and gender, BMI, or mMRC score. Conclusion: Older age, respiratory failure, high symptom burden, and abnormal arterial blood gas parameters are significant predictors of poor outcomes in patients hospitalized with AECOPD. Early recognition of these factors is essential for effective risk stratification and optimization of clinical management.
