CHANGES IN PARENTAL ANXIETY BEFORE AND AFTER PEDIATRIC CIRCUMCISION: A STUDY USING STAI AND VAS-A SCALES

Abstract

Objective: To evaluate changes in parental anxiety levels before (T1) and after (T2) circumcision in children with phimosis, using the standardized STAI and VAS-A scales. Methods: A descriptive study was conducted at the Department of Andrology and Urology, 108 Military Central Hospital (January 2025–July 2025). Participants were 57 parents (35 mothers, 61.4%; 22 fathers, 38.6%) of boys aged 3–10 years undergoing circumcision. Trait anxiety (STAI-Trait) was assessed once at T1, while state anxiety (STAI-State) and VAS-A were measured at both T1 and T2. Data were analyzed using paired t-tests, with statistical significance set at p<0.05. Results: The mean age of the children was 6 years, and the mean parental age was 35 years. Most parents lived in urban areas and had at least a high school education. The majority of circumcision indications were simple phimosis. The mean STAI-Trait score was 42.3±7.6. STAI-State decreased from 48.3±7.2 at T1 to 35.6±6.4 at T2, a reduction of 12.7 points (p<0.001). VAS-A decreased from 7.2±1.5 to 3.1±1.2, a reduction of 4.1 points (p<0.001). Circumcision was performed safely, with an average operative time of 25.5±6.8 minutes. No intraoperative complications occurred. Postoperative complications were observed in 7 cases, mostly localized swelling at the surgical site. Conclusion: Circumcision for pediatric phimosis significantly reduces parental anxiety after surgery. The use of standardized anxiety assessment tools such as STAI and VAS-A enables quantification of the effectiveness of counseling and psychological support interventions.
