PALLIATIVE CARE NEEDS IN END-STAGE KIDNEY DISEASE PATIENTS UNDERGOING MAINTENANCE HEMODIALYSIS IN THUY NGUYEN GENERAL HOSPITAL, HAI PHONG

Abstract

Objective: This study aimed to describe the palliative care (PC) needs of end-stage renal disease (ESKD) patients undergoing maintenance hemodialysis (MHD) at the Hemodialysis Unit, Thuy Nguyen General Hospital. Materials and Methods: From December 2024 to April 2025, we conducted a descriptive study. We interviewed 72 ESKD patients undergoing MHD using the Vietnamese Palliative Outcome Scale (VietPOS). Results: Physically, the most common and severe symptoms were: fatigue (79.2% prevalence, 65.3% severe), anorexia (73.6% prevalence, 56.9% severe), and pain (59.7% prevalence, 51.4% severe). Weight loss (51.4%) and nausea/vomiting (50%) were also prevalent. Regarding psychosocial issues, the prevalence and severity of problems were: lack of information about the illness (66.7% prevalence, 48.6% severe), illness-related anxiety (26.4% prevalence, 20.8% severe), lack of emotional support (50% prevalence, 37.5% severe), sadness (37.5% prevalence, 22.2% severe), financial worries (50.0% prevalence, 36.1% severe), and feeling abandoned/stigmatized (20.8% prevalence, 5.6% severe). Spiritually, 57% of patients reported issues with their sense of peace, with 37.5% feeling no peace at all. The top five most severe issues, based on mean score analysis, were: fatigue (3.19 ± 2.00), lack of appetite (2.75 ± 2.06), lack of information (2.43 ± 2.14), pain (2.40 ± 2.17), and emotional support (1.93 ± 2.17). Conclusion: ESKD patients undergoing MHD have high palliative care needs across all physical, psychological, social, and spiritual domains. Integrating palliative care into the management of ESRD patients on MHD is crucial to alleviate suffering and enhance their quality of life.
