TREATMENT ADHERENCE IN PATIENTS WITH CHRONIC GOUT AT VINH PHUC GENERAL HOSPITAL

Abstract

Objectives: To analyze the current status of treatment adherence and Glucocorticoid (GCs) use in patients with chronic Gout. Methods: A cross-sectional descriptive study was conducted on 95 chronic Gout inpatients at Vinh Phuc General Hospital from July 2024 to July 2025. Treatment adherence (medication, diet, and hydration) was assessed using the Morisky Medication Adherence Scale (MMAS-8). Results: The mean age was 63.67 ± 13.86 years; males accounted for 95.8%; 56.8% presented with tophi. The rate of medication adherence was moderate (53.7%), whereas adherence to dietary and hydration regimens was very low (29.5% and 30.5%, respectively). The prevalence of GCs use was high (46.3%), primarily via oral and combined routes; notably, 43.2% of patients engaged in self-medication and self-adjustment of dosages. Only 52.6% of patients maintained urate-lowering therapy (ULT). A high frequency of acute gout flares (≥5 times/year) and comorbid hypertension were significantly associated with poor adherence (p < 0.05). Conversely, family support and knowledge of the treatment regimen were critical protective factors, significantly associated with good adherence across all three domains: medication, diet, and hydration (p < 0.05). Conclusion: Treatment adherence in chronic Gout patients remains limited, particularly regarding lifestyle modifications. The abuse of Glucocorticoids and inadequate medication management are alarming issues that require intervention.
