PREVALENCE OF RISK FOR OBSTRUCTIVE SLEEP APNEA AMONG INPATIENTS AT NHAN DAN GIA DINH HOSPITAL

Abstract
Background: Obstructive sleep apnea (OSA) is a common sleep-related breathing disorder. The diagnosis of OSA is recommended to base on the gold standard of polysomnography or respiratory polygraphy. STOP-Bang is the most widely used questionnaire for OSA screening, and has been proven its value through many studies on preoperative subjects and sleep units. Objectives: We used this questionnaire to determine the proportion of inpatients being at risk for OSA, and applied respiratory polygraphy to confirm the diagnosis, thereby estimating the burden of disease in the hospital. Methods: This was a cross-sectional study. A total of 129 patients selected from 694 inpatients by stratified random sampling were asked to complete the STOP-Bang questionnaire, while demographic data were recorded. A score of ≥ 3 on the questionnaire indicated a risk of OSA. 30 at-risk patients were randomly drawn to complete respiratory polygraphy, we noted apnea-hypopnea index (AHI) and OSA rate. Results: A total of 64 subjects (49,6%) had a STOP-Bang score of ≥ 3, of which 16 subjects (12.4%) had a score of ≥ 5. Analysis of 30 at-risk cases that had respiratory polygraphies meeting quality criteria, 18 patients (60.0%) and 9 patients (30.0%) had an AHI of ≥ 15 and ≥ 30 events per hour, respectively. Frequency of obstructive sleep apnea with OSA definition using an AHI of ≥ 15 was estimated to be 29.8% (95% CI: 21,9%–37,7%). Conclusions: The frequency of OSA determined by STOP-Bang score and respiratory polygraphy in inpatients is generally high. Our findings indicate needs for a broader application of the STOP-Bang questionnaire to screen for OSA in hospitalized patients and equipment of respiratory polygraph or polysomnography to confirm diagnosis in at-risk ones.
