HEALTH-RELATED QUALITY OF LIFE AND ITS ASSOCIATED FACTORS AMONG OLDER ADULTS AFTER ISCHEMIC STROKE: A CROSS-SECTIONAL STUDY

Abstract
Background: Ischemic stroke can substantially impair health-related quality of life (HRQoL) in older adults. Identifying associated factors is essential to guide post-stroke care and rehabilitation. Objectives: To assess HRQoL and identify factors associated with HRQoL among older adults after ischemic stroke. Methods: A cross-sectional study was conducted on 146 older adults one month after ischemic stroke. HRQoL was assessed using the EQ-5D-5L. Functional status (ADL, IADL), depressive symptoms, stroke severity (NIHSS), comorbidity count, sleep quality, and family support were evaluated. Univariate and multivariable linear regression analyses were performed. Results: The mean age of participants was 79.13 ± 10.65 years, and 56.2% were male. Substantial impairments were observed across all EQ-5D-5L dimensions, particularly in mobility, self-care, and usual activities. In univariate analysis, better sleep quality was associated with higher HRQoL. In multivariable analysis, higher levels of independence in basic and instrumental activities of daily living were independently associated with better HRQoL, while depressive symptoms were independently associated with poorer HRQoL. Stroke severity and comorbidity count were not independently associated with HRQoL. Conclusions: Among older adults after ischemic stroke, functional independence and depressive symptoms were the key factors associated with HRQoL. Interventions focusing on functional recovery and mental health may help improve quality of life in this population
